Dear Parents,

I hope you are enjoying your summer! To help make the transition back to school as smooth as possible, here is a list of all the forms that we will need in the office by Monday, August 2nd. (Click to access PDF versions of the forms.)

• Medical Forms: Physical – anyone new to our school, all Kindergarteners, all 6th graders
• Sports Physical – anyone participating in sports
• Eye Exam Form – all Kindergarteners, anyone enrolling for the first time in a school (except preschoolers)
• Dental Exam Form – all Kindergarteners, all 2nd graders, all 6th graders
• Joliet Diocesan Information Form (each student)
• Emergency Information Sheet (one per family)
• Acceptable Use Agreement Signature Page (separate form for each student)
• Safe Environment for Children Signature Form (parents’ signature)
• PEAK Forms (if necesssary -- Information Registration)
• Milk Order Form (if desired)

Additionally, the bus schedule and our school supply lists will be available soon. For those families who pay tuition in full, tuition is due by August 25th. All other families should enroll with FACTS Tuition Management (the direct link is on the website for your convenience).
