The following forms are usually filled out in the spring/summer:

			St. Irene '08-'09 Tuition Agreement

The following forms must be completed and turned in on registration day:

			Joliet Diocese Student Information Form
			St. Irene Emergency Information Form

Based on your child's grade or your needs/preferences, you may also need to bring these forms:

			Permission Denied to use Personally Identifiable Student Images and Work (optional)
			State Health Form (for new students and students entering grades K and 5)
			Medical Permission Form (if students are required to take medications during school hours)
			Dental Examination Form (for new students, and students entering grades K, 2, and 8)
			Parent Service Hours Form (for new families)
			Sports Registration Form (for new families with students in grades 5-8, if they wish to play sports)
			PEAK Registration Form (only required for those who will be participating)

Required reading before registration -- the Diocese has asked that every parent review the following materials before the start of the school year. A signature will be required at registration, confirming that you have read the documents.

			Videotaping/Photography of Students Agreement
			Acceptable Use Agreement for Internet Access and Related Technology Use (for students -- read this document first)
			Acceptable Use Agreement for Internet Access and Related Technology Use (for students -- sign and return this form after reading the above document)
			Staff/Volunteer Agreement for Internet Access and Related Technology Use
			Safe Environment for Children Brochure (legal sized) -- the school is required to document that you have received and reviewed this information.
