The following forms are usually filled out in the spring/summer:

St. Irene '10-'11 Registration Form and Tuition Agreement

The following forms must be completed and turned in to the school office by Friday, August 15::

			Joliet Diocesan Student Information Sheet
			St. Irene Emergency Information Form
			Acceptable Use Agreement for Internet Access and Related Technology Use (for students -- read the entire document then both student and parent sign page 4 and return)
			Safe Environment for Children Signature Form (for parents -- sign and return this form after reading the document in the Required Reading section below)

Based on your child's grade or your needs/preferences, you may also need to bring these forms:

			Permission Denied to use Personally Identifiable Student Images and Work Form (optional)
			State Health Exam Form (for new students and students entering grades K and 6, and all students in athletics, grades 5-8)
			Medical Permission Form (if students are required to take medications during school hours)
			State Dental Examination Form (for students entering grades K, 2, and 6, and any students new to Illinois)
			State Vision Examination Form (for Kindergarten students and any students new to Illinois)
			Parent Service Hours Form
			'10-'11 Sports Registration Form (for families with students in grades 5-8, if they wish to play sports)
			PEAK Registration Form (only required for those who will be participating)
			Extended Day Child Care Agreement (also required for children attending PEAK)
			Milk Order Form

Required reading before registration -- the Diocese has asked that every parent review the following materials before the start of the school year. A signature will be required at registration, confirming that you have read the documents.

			Videotaping/Photography of Students Agreement
			Staff/Volunteer Agreement for Internet Access and Related Technology Use
			Safe Environment for Children Brochure (legal sized) -- the school is required to document that you have received and reviewed this information.
